- Professional assessments – A thorough assessment allows the advocate, or care manager, to understand your family’s needs, develop a customized plan, and deliver services that address your specific situation. Assessment interview questions cover a range of issues related to your loved one’s health and living situation, including everyday activities, health history, nutritional status, safety concerns, cognitive needs, depression, finances, and insurance.
- Planning – Our customized Plan of Care includes the results of our assessment, recommendations, and referrals for local community support options. Your advocate will explain details of the plan, the reasoning behind plan recommendations, and what you and your loved one can expect. They will then help to prioritize the list and guide you in its implementation.
- Plan facilitation – Once services and arrangements are in place for you or your loved one, regular monitoring is important. Our professionals will also provide frequent reevaluations to make any necessary adjustments to the care plan.
- Coaching and guidance for family caregivers – As advocates, we provide ongoing support and education to family caregivers to help ensure quality of life and optimum care for your loved one and reduce worry, uncertainty, and stress for everyone involved.
